Adding the same form to multiple pages in a document

Hello.

I have a form that I have created. What I wanted to know if there is a way
to have the blank form automatically add itself to new pages in a document. I
need the form's format but the information changes for each page.
 
Look into the AutoText feature. There is no automatic way short of a macro.
If your AutoText entry is formatted so that the paragraph formatting of the
first paragraph in the entry is set to have a page break before, this will
get you most of what you seem to want.

Hello. I'm not sure what I should do with the AutoText entry...especially
since the user would have to remember what to type.

What kind of macro should I consider? I have never used a macro before.
