Is it possible to monitor/log changes made to Active
Directory?

What type of changes? There are lots of things to view....

Take a look at the post from Tim Hines on 02/29/2004. It includes the two
major types "Audit account management" as well as "audit Directory Service
Access".

That should be a good start.
