accumulating entries

I am setting up a report that counts errors. Is there a way to count ho
many times a name appears on a report, and return that in a cell. Fo
example:

john smith - no notes
bill wert - wrong price
corry wells - no notes
john smith - no notes

Can I have the above data be consolidated on a sheet that will say:

john smith - 2 errors
bill wert - 1 error

etc.
