Access Database Template - Customer Survey

Has anyone developed a template for customer surveys? Just basic so I can
store each submitted survey as a separate record for reporting purposes.

Some experienced users and developers, after evaluating Steve's answers
here, have opined that no fee could be sufficiently modest, given the

Stevie, I am starting to worry about you. For years you have ignored the
requests for you to cease, but you continue. Sometimes totally oblivious to
