Access Database for Tracking Drug Reactions

Having trouble building an acess database to store drug
reactions for patients, print annual reports & letters to
physicians. Also needs different levels of access to the
forms & reports. Is there a web site w/ templates or a
sourse to have the DB built for me?

Not sure about any templates available, but I'd be happy to build it for you
for a reasonable fee....
