Access based questionaire

We're in the process of establishing a new not-for-profit endeavor in the
former Soviet Union and have a forty page paper questionnaire/survey that
we'd like to computerize...stand-alone and Internet...using Access as the
underlying database.

Most questions are simple "select one answer from several" but there are
many questions having multiple responses the user can check.

I'd appreciate any suggestions as to books or online sources on creating
Access based questionnaires/surveys.

Don't lose sight of the fact that how you accept the input and what you do
with it are (usually) two different things.

Use Duane's application as a starting point. You should be able to integrate
weighting into it without too much difficult.
 
Thanks...been googling without a lot of success but my criteria may have
been to complex...will try the more simplified you recommended...thanks.

Again thanks much. That link offers an application which, although easy to
use, doesn't support scoring of weighted input.

I guess I'll end up developing the application so I'm looking for some
tutorials/papers rather than re-inventing the wheel.

Am not averse to spending some $$ for an application, if I can find one that
does what we need at a reasonable cost. Would save me a significant chunk
of time :)

I am surprised that I'm not finding any substantive tutorials...but I might
be looking in the wrong places...wouldn't be the first time.
