Would like to see a form(template) that helps for an individual to compliment
co-workers who have been a big part of someones life for a great number of
years

I somehow doubt that you want a form in an Access database, the purpose of
this particular newsgroup.

I'd recommend making your suggestion in a newsgroup related to Word.
