word checkboxes in tablet setting

Is there a way for the word document to respond to tablet checking of checkboxes and drop down menus?

For example, we have a physician using a word document who would like to click on at checkbox and have it record a yes or no or to be able to select an item from a dropdown list on the form.

Can anyone direct me to a document explaining how to do this if it is possible
