word 2002 form fields do not convert to html

I am making an ms word form and am having trouble converting the form fields to html. I am trying to use vba to save the document in html format (this is because it is easier to distribute electronically in this format). I have been able to save it as html successfully, and the entire document looks fine except for the form fields. The text form fields turn out great, but the drop-down fields and check-boxes disappear! I have no idea what is going on and have tried very unsuccessfully to get it to work or find a solution elsewhere.

Any ideas? Any solutions, possible causes, or help of any kind would be greatly appreciated!

( I am using Word 2002 with Windows 2000 )
