Win 2k Server TS and CALs

Our TS crashed and we reloaded it, and I'm trying to install the CALs. I've
got the license code sheet and the Microsoft License Pak sheet, but when I go
to install the CALs it's asking for an authorization number that I can't
find. Do I need to get more CALs or will Microsoft reissue an authorization
number?

How did you purchase your TS CALs, and which license program did you
choose in the Install Licenses Wizard?
