Wierd login dialog box

I have Exchange 2003, and am using Outlook 2003. I have a user that would
get a wierd login dialog box every time she tried to open Outlook. It acted
as though it were trying to open her mailbox manually by using a login dialog
box, except it wasn't her account. It was a user (L HAYES) that has no
identity on this domain.

Is this a virus or malware or what?

But it used to be a user in your domain?
Have you checked her account configuration?
