Why are known forms missing from printer paper size list?

In Word 2003 I'm trying to print on 4x6 index cards but the output is shifted
left so I only see the right side of the printed text. I specified a custom
form in "paper size".

Related question:
In Word 2003 File/Page Setup. Select Paper tab. The pulldown list under
Paper Size does not include many of the forms which are shown in the list of
forms for the printer server (Control Panel/Printers and Faxes/File/Server
Properties). I have an HP Laserjet 5L installed.

I'm trying to print recipe cards.

The laserjet aligns from the middle. Try the envelope feed.
Word will only print paper sizes that the printer driver will allow.

I solved the problem by rotating the index card in the paper bin and printing
it in landscape mode. I specified a custom form 6" wide x 4" high on the
printer setup page.
