What could lead to missing icon in custom toolbar button?

I followed the instructions described in MSDN and entitled "Adding
Toolbar Buttons":

It used to work fantastically.

Then I tried to improve the icons. Among other things, by adding the
optional 20x20 16-color icon.

Now, in one system, installation results in blank button (neither icon
nor hoticon). In another system installation results in the hoticon
somehow appearing in Messenger's button.

Any ideas how to troubleshoot this? I am currently clueless.

Hi Victor,

Reset your toolbar buttons (Customize Buttons dialog - Reset button)

The registry key which holds the buttons ordinals has a maximum length(I
think) and when you exceed this limit the button icons get all mixed up
(IE6, haven't seen it on IE7 yet, the IE7 buttons display algorithm is
different, haven't worked it out yet)

On my IE6 test machine I have 50+ custom buttons and have seen this. I don't
think there is a fix, so I guess you would call it an IE6 feature.

Beware that IE7 buttons do not accept environment tokens in the pathnames to
icons (eg. %windir%) whereas IE6 does.
