volume control icon missing from taskbar

On-going problem with my Dell 8300 with a Creative Audigy 2 sound card.

Recently (?) neither the speaker icon from control panel nor the creative
volume icon from the audigy software will remain showing on the taskbar
after activating one or the other and then rebooting. They will show up
when activated from the control panel or from the surround mixer program
from creative. But after rebooting, they just disappear. Problem didn't
exist when purchased computer and can't say how long ago it started showing
up. Have not added any hardware to system since purchasing.

CTSysVol is checked and active in the MSConfig startup list
Have clicked on the "display icon on taskbar" in surround mixer from
Creative SoundBlaster. Checkmark disappears after rebooting
Have clicked on the "show icon on taskbar" from the audio/sound setup in
control panel. Checkmark disappears after a couple of reboots.
Have done a repair/ reinstall of WinXP Professional
Have updated the audigy software from Creative
Have tried activating one, or the other, or both of the icon selection boxes
at various times
Icons don't always show even if the checkboxes are marked as active in
control panel and surround mixer
Have tried checking and unchecking "hide inactive icons" from the settings
page from the startup menu.
When looking at icon status after checking "hide inactive icons" I have
tried checking "always show" as the preference. Frequently when going back
to this dialogue box the listed program has just a generic folder as the
icon instead of the actual program icon. But doesn't seem to have any
effect on the display in the taskbar.

Anyone else have an idea?
