View a published form

When I try to publish a form and run a test, the reciever see's only two
fields. What am I doing wrong?

Where did you publish the form? How many fields does the Read page of the form display in design mode?

We have a department within our company the publishes all our created form
for all employees to see. I was told to look at my form in design mode and
confirm that I had the custom fields on both the 'Compose' page and the
'Read' page. I resent the form and this was their response...It's still not
working right. I only get two field values returned (Siebel ID and
Certification Contact Name).

Not sure what to do now. I have at least 15 field to choose from on my
form.

You still didn't say where the form is published. Unless it is published to the Organizational Forms library on the Exchange server, it's not going to work.

On the read page, did you check each control to make sure it is bound to the right Outlook property?

Also, if you increment the version number on the Properties page each time the form is published, you'll be able to troubleshoot using the Help | About This Form dialog to find out what version the user is actually seeing.
