Video Card- What to check next?

Trying to help my friend by remote assistance and Card is Geforce 4 TI 4200
This problem just started and I am starting to think maybe card failure. Any game using D3D or opengl will lock up the computer but no event or dump log. Screen just starts turning into as she calls it "shards of colors". can not move mouse or get any keyboard key to work
I have had her uninstall/reinstall drivers in safe mode and then test display in dxdiag and all tests pass
I had her run a memory test which passed too. In Half Life game you can select from opengl, D3D, or software mode and only software mode will run when before she could run any mode
So, I am wondering what to check next as tests come out fine and I am sure drivers are not the problem now
I have an opengl tester to try too but have a feeling all will be ok there too

Use the latest video card drivers found @ www.nvidia.com

Install the latest chipset drivers for her motherboard chipset.

Check to see that the power supply is prociding enough power for her system.
With an nVidia Ti 4200, power supply should be rated 350 watts or more.

Make sure that the computer is not over-heating. Check power
supply/cpu/case fans and replace as needed. Also, if no case fans are
installed, install some. Fans in the front should "pull" air in from the
front and rear fans need to "push" the air out from the case.
