User input url in Front Page 2003

On my website, I post client image proofs to be viewed by the client. I post
the proofs in a subdirictory with a unique name supplied to the client, so
they can see them by typing my web address plus the subdirectory name. For
instance a client named smith would type the address
www.mywebsite.com/smithproofs to see his proofs. How can I create a page
within my website that ask them to input their unique name and the form then
opens their url address that has their unique name as a part of the url
address. For instance, Smith would input "smithproofs" in a field and click
a submit button and he would then be sent to
www.mywebsite.com/smithproofs/index.htm.

Password protecting part of the web does not appear to create a way I can
have a client input their name on a page and have that page transport them to
their url address which is made up of my website address plus their name as a
subdirectory.

This method can be accomplish with a number of JavaScript Password Scripts that you would have to
modify to fit your requirements.
