Download CCleaner from www.ccleaner.com. Boot into safe made, clear out
your temp files with CCleaner. Download Ewido from www.ewido.net. Install
it, do the update and then scan your system. Let me clean out whatever it
finds and reboot. Check to see if the file or random file(s) are still
there.

No ONE antispyware (AS), nor AV program for that matter,
WILL find everything. Using multiple applications helps
ensure that everything is detected. But, only use the
real-time protection features of one AS and one AV
application at a time, otherwise they can, and LIKELY
WILL, conflict with one another.

I've run a scan with Spybot 1.3, and it found absolutely
NOTHING. I then ran a scan with the trial version of
Giant AntiSpyware, now MSAS after MS bought out Giant
Company, and it found about 3 Trojans, a keylogger, and
numerous other things that Spybot didn't even notice!
Just goes to show you, you MUST use more than one
application to find everything!

I had already started to dislike Spybot as its
Immunization feature required the user to launch Spybot
EVERY TIME that they needed to change a setting in IE,
and turn off the feature! Talk about ABSOLUTELY STUPID
programming! After I found out what all it missed, it
uninstalled it and then downloaded Ad-Aware. I've been
using Ad-Aware for the last 8 months AND MSAS for about
the last 5 months. Both are MUCH better programs than
Spybot!
