Unable to create a new text doc using right click

I cannot create a new text document in any folder
using right click - can only create a new folder or a new
file.
Please help

See if this works; copy and paste this into a blank text document and save
it as ' text.reg '. Right-click the reg file and ' Merge '.

Please be a bit more explicit. Do you mean that the option to create a new
text document is not there in the right-click context menu, or do you mean
that the option IS there, but nothing happens when you click on it?
