Track Changes

My assistant and I often work on the same document. We don't like or use
track changes, yet, sometimes the track changes feature seems to come on by
itself, without either of us having turned it on.

What steps can we take to make sure it is off all the time, and doesn't come
on unless, for some reason, we would want it on?

Don't accidentally type Ctrl-Shift-E (presumably when you're trying to
execute some other shortcut).
