Title bar missing

One of our users had their Excel title bar missing. Therefore could not
minimize, maximize, close etc. with the buttons.

It was fixed by using the Excel Detect and Repair function but I was
wondering what that could have been?

I've seen this many times, usually as the result of the Worksheet window
being move up, so that the title bar gets obscured by the Formula Bar (or
toolbars) of the Excel window. An easy fix for this is to rearrange the
windows.

Select "Arrange..." from the Window Menu.
Select "Tile" (or any option here will work)
Click OK

If that were the case it would have been simple. Unfortunately it was not as
simple as that. It's the first thing I thought about too. So if anyone has
seen this before pls. I still need an answer.
