Task Question - Sharing & Customization

I work for a medium sized company that is struggling to process new-hires in
a timely manner. For each new member of the staff, we generally have to
process them through five or six separate depts in order to get their email,
security codes, payroll paperwork, etc. all taken care of.

Ideally, we'd like to have HR (or the new Employee's mgr) initiate & then
send out one shared task related to that individual that each dept. could
then process & update.

The task itself - would need some level of customization - check boxes, pull
down menus etc., so that specific profile and security settings for each
employee could be outlined within the task before it goes out.

Once completed, the form could then be saved to a digital employee file or
printed to hard copy for the employee file.

Can anyone tell me if this is possible?
