Table of Contents link to referenced document

This question has 2 parts. I know that when creating a table of contents,
pages are automatically linked to the heading within that document. However,
in my case, I'm using the RD field code because I have multiple documents
that are being referenced in my TOC. So..

1) Is there a way (besides manually inserting hyperlinks) to click on a page
link in my TOC and be taken to one of my "outside" documents?

2) If so, how?

I can't find anything referring to being able to link a TOC that uses the RD
field. Only mention of hyperlinks when the TOC is within the same document.
