Shortcut key to open program doesn't work

I have certain applications that aren't in my start menu programs list. If I
right click on the .exe file of most of the application they do not give me
the option to be able to specify a short key (ie, CTRL+ALT+X). However, if I
create a shortcut/alias of the .exe by doing "file > create shortcut" I am
able to right click this shortcut and specify a shortcut key, it just
doesn't work when I do, even though double clicking the file finds the
target and opens the .exe.

any ideas?

SYMPTOMS
You are unable to run a shortcut by pressing its shortcut key.
CAUSE
Shortcut keys work only for shortcuts on the desktop or in the Start menu hierarchy.
