Shared Borders don't show, page appears twice

I am having problems with my FP2002 designed site at
http://www.conferenceteam.com. It previews perfectly on
my local PC and also works if I upload it by FTP. However
to get the forms to work I need to upload it by HTTP and
this means that I get the mess that is currently showing.

The shared border (for navigation) doesn't show and the
body of the page appears twice, once without graphics and
then again with graphics.

My host is 1and1.co.uk on Unix servers supporting FP2002
extensions.
