Set file associations, commands and icons.

Windows Explorer -> Tools ->File Associations tab is gone under
Windows Vista. The only thing available is to set default 'open'
application for 'exising' extensions. Now i can't
1) change icons for specific extensions
2) add custom commands like 'Edit' or 'Open with notepad'
3) remove extensions (that created by uninstalled applications and I
don't want)
which had been possible through Windows 95 through Windows 2003.
Should I used some 3rd party applications to do these? Or am I
supposed to manually edit the registry entries with regedit? What
options do I have?

Rubbish, isn't it? Best solution I have found so far is this:
