remove addresses

Using distribution lists, a user updates lists, adding and removing names.
From time to time, an email goes to an old deleted address. We've tried to go
in, find it and remove it, and cannot find it.

Any ideas?

"A user updates list" means what exactly?
DL's are stored where?
How is the DL selected as the recipient?
