Printing Forms in Outlook

I created a rather nice looking form that I use for sharing information. It
shows well electronically but when I send it to print I don't get what's on
the screen. I get an alphabetic list of the fields in the form and their
contents. My question is: -- Is there a way that I can print the form so it
looks the same on the page as I does on the screen?

the limitation that Outlook forms don't support WYSIWYG printing. The Word
template technique is the most flexible and highly recommended.

FYI, there is a newsgroup specifically for Outlook forms issues "down the
