Once a set of changes to a Word document has been accepted and new changes
are being tracked, can you go back and see >all< changes made (i.e., both
accepted and unreviewed changes)? Or are tracked changes completely lost once
the changes are accepted and the document saved?

According to my knowledge, this last assessment is true, yes. You would
have to keep a copy of the document before accepting the changes in
stage 1 for reference.
