positioning of comment balloons

I'm using Word XP and having problems with the positioning of the comment
balloons when I insert comments. Sometimes the balloons appear in the
right-hand margin next to the associated text (which is what I want to
happen) other times all the balloons bunch up near the top of the page and
only the red marker brackets are left in the body of the text which makes
following the comments and their context very hard to do.
It's an inconsistent problem. Some documents are fine, others not. I have
one document where the comments are correctly positioned on page one but
bunched on the remaining pages.
Any ideas?

Does the bunching appear to be caused by crowding in the margins (too many
balloons to fit where they are referenced)? If so, perhaps increasing the
percentage of margin allowed for the balloons would help.

The first balloon on a page will appear at the top of the page as soon as I
"insert comment" before I start typing in it.

Well, that definitely doesn't sound called for, but I can't think what would
be causing the problem.
