Outlook Custom Form

I created an Outlook Custom Form and runs on Outlook
2003 clients. It was working for a week. Now when a
users try to use this form, it comes up as an Outlook
Form (displays only standard outlook fields). The orginal
custom form (windows) does not come up.

Any ideas?

What would have caused this?

If the user has Outlook 2003 SP2, they'll need to launch it with the Tools | Forms | Choose Form command rather than running a saved .oft file.
