Is it possible to have a form set up so that when specific dates are entered
in a form that those dates are automatically reserved in my Outlook Calendar?

You would need to ask this in the Outlook newsgroup, as FP has no functions that directly interact
with Outlook.

While it is not possible for your website visitor to automatically
book your calendar, what you could do is create some scripts
on your server to:
#1 Create and save an ICS file on the server
#2 Send you an email with a link to the ICS file
