Old User Accounts Taking Licneses

I'm have an issue with accounts that have been deleted
from my domain, still showing up and taking CAL's in
License Mgr.
Even after revoking all the licenses these old accounts
still eventually show up and take a CAL again.
My client has a high turnover and there are constant user
accounts being deleted and created, this problem is
continually causing him to run out of CAL's even though
he's covered for what he needs.

Any ideas how to clean this up?
