No Longer Sorts by File Name

My computer(s) have always sorted files by name. Recently, and for no good
reason that I can find, my files default to "sort by date modified" in all
situations where i would open up a folder or dialogue box. It is driving me
insane as I name my files using additional letters at the beginning of file
names in order to create my own personal hierarchy.

How can I change this back permanently. I know I can right click and select
"sort by name" each and every time I open a folder, but I want the default to
return to "by name" so I won't have to right click.

Click on view
click details
click on the name column label to sort by name
click on tools
click on folder options
click on the view tab
click the apply to all folders button
click yes on the popup dialog
click ok
