I had a similar problem until I performed a clean boot
in XP using msconfig. By disabling all of the startup
entries (uncheck boxes - disable all). I was able to
verify each startup executable one by one (multiple re-
boots).
In my case I had two instances (I'm not sure why) of
CFTMON.exe checked and starting at the same time. I left
one unchecked, and restarted the OS and the problem was
solved.
A little about CTFMON:

Note : Prior to taking the above action I had upgraded my
sound card drivers, and system bios. There were no
unresolved issues in the device manager or
in the Application and System event logs.
Running DXDIAG at the run box validates the error in the
test direct sound section, and de- accelerating
the 'Hardware Sound Acceleration Level' does not help.
After problem is fixed there are no error messages
regarding Hardware Buffering.
