Missing Desktop Icons and Startmenu after reboot

I have a pc, WXP Pro Sp2 that was infected with various viruses and spyware.
I believe that I have successfully removed them all, I've ran several scans
using 2 different scanners, everything has come back clean.
I've also fully patched my system, as of 10/11/2007, all critical and
recommended updates and IE7.

When I boot my pc the desktop fails to load completely. The backgroud is
visable, but no start menu or button and no icons.
I am able to bring up the task manager and by running "explorer" on the
command line most of the desktop appears. Not all Icons in the notification
area appear.
After I reboot the machine after running explorer most of the times the
desktop will appear normally, but not always.

Has anyone seen this and have any recommendations as to what I can look for
or into more?
