Minutes of meeting

I am looking for a good template of a "minutes of meeting" form to be used in
a club. I am aware of the templates available at Microsoft.

Are you aware of the agenda wizard? While it says it is for an agenda, it
will also prepare a detailed minute sheet at the same time.
