mailto feature of Submit control

Hi,

I am trying to build a web survey form in Word X for Mac. I am trying to
use the "mailto" feture of the "Submit" HTML object, but it doesn't seem to
work. Could someone please suppy examples of the required information in the
"Action", Encoding" and "Method" entries of the Submit button? Or point me
to a template that does this? Thanks very much!
