Internal E-mail ending up in Spam folder

Some of my users are getting some of other users' e-mail in their Spam
folder. None of the users (or anybody else) has been configuring Spam
settings in their Outlook. We have an external and very well functioning spam
filtering solution, and do not filter spam internally. There is nothing
unusual with the e-mail messages ending up in the spam folder. They are
tagged [Spam].

As fair as I know, this phenomenon has appeared recently and spontaneously.

All are using Windows XP SP2 and updated Outlook 2003. It's an updated
Windows Active Directory environment with updated Exchange 2003.

Any help much appreciated; due to the key words involved, it's very
difficult to google effectively on this problem.

It is very few e-mail messages ending up i the spam folders (most of the
users have gotten on or another). Also, a handfull external senders' e-mail
messages have ended up in some users' Spam folders. -- Those messages are
very far from spam, and should not have been filtered by any application.

If they're tagged that way, then something other than Outlook is doing it
because Outlook doesn't ever alter the messages it considers SPAM.

So what was the solution?

I'm afraid I hasn't got the solution yet. Your information was invaluable in
the sense that I was banging my head against Outlook, and you freed me from
that ordeal.

What I've discovered is that e-mail ending up in the Spam folder (or "Junk")
from internal senders often fits in a pattern:

No subject
No message
Attachement

But some e-mails end up there from external senders too. I haven't got any
filtering in Exchange, and the external spam solution doesn't tag spam
messages.

We have got an old internal solution -- MailWall -- which still is out
there, but the internal messages aren't routed through that server.

Thanks for your interest, I will get back when/if I find a solution.
