Insert --> Object, create from file. Click Browse. Navigate to your file.
Before closing the create from file window, check the box on the right side
that says "Display as icon". (FYI: Display as icon isn't available to be
clicked until you have selected a file. For me, that means I forget where it
is quite frequently.)
