Importing a Form

I have an existing PDF from that I would like to import into an Outlook form.
Is that possible? It's a Time Off Request that we want employees to be able
to access from Outlook and just send in instead of printing off and wasting
paper.

No, that's not possible. There is no correspondence between PDF and Outlook
forms, nor are there any conversion tools.
