I need word to always open my form

Hello,
I'll try to explain my problem in english the best that I can.
I have created a word document. How can it be possible each time I open Word
to open that fax document if I want to write a fax. Of course I want it to
open at it's original form with blank every textbox I have inside that fax
document. But I don't want the fax document to be opened every time I open
MS-Word, I just want to have the oppertunity to open it whenever I need to
write a fax. So I need to chose someway, or maybe click something to open my
fax document.
I hope you can understand what I need.

Save your form with the default text and layout but without the data as a
template. You can then open new documents based on this template.
