Hyperlink the document

Hi Everyone,

It is possible to Hyperlink a form to the email id. Let me explain;
I have created a form on a word document. Can i create a link at the bottom
of the form, which will help the person after completing the form to send a
mail and the document is attached automatically and he has to click the send
button the outlook.
