How to create the entity for a questionnaire survey database...

I need to create an Access application to capture the response of the
questionnaire surveys. The questions in the surveys that need to be captured
appearing in so many different formats (question types such as single choice
out of multiple choices, multiple choices out of multiple choices, single
text answer, combination of multiple choice and text answer, yes/no question
and so on, so far I identify 15 different favours of the question types )
and the question could nest in several levels in the main question eg.
question, sub-question and sub-question and so on. Please I need some help
first to create the back-end to support this kind of the survey question.
Please if you know any books and any Access survey samples would be much
appreciated. Thanks.
