how to append a form (template) to an existing doc

I have a from (saved as a .dot template) and I want to append it to the end
of a .doc I have as an appendix. Then I need to update the TOC to show the
appendix. Can anyone give me the steps?

In Word 2003, using marked text - not styles. Mark the text, then go to index
and tables, and select the options that suit your needs.
