How do I write an amendment letter?

i need to send a letter to an employee for an amendment w-2 due to a payroll
error in 2006. We process a check for him by mistake and we did a reversal
from the bank but we never delete the transaction from his file and now his
w-2 is wrong. I did a amendment for his w-2 2006 and he is going to receive a
W-2C to be able to fix his taxes for last year.
