How do I link form fields in Word

I use forms in Word with a number of form fields. The information is on a
number of pages, and some of the same information needs to be at the top of
each page (ie name and date of birth). I would like to input the information
on the first page and then have the other fields automatically fill in with
the same information rather than typing the same info at the top of each
page. Thank you for any help.
