How can I remove 3rd party menubar in Outlook E-mail windows?

I installed a trial 30-days CRM software which managed to embed some of the
e-mail function into Outlook E-mail windows. As results, the menubar has one
extra "ACT!" between Windows and Help menubar

When I uninstalled the software, the extra menubar stays in-between,

How to remove it?

I am using Outlook 2003 SP1

See if anything of the add-in is left here;
Tools-> Options-> tab Other-> button Advanced Options...-> button COM
Add-Ins/Add-In Manager

If nothing of the add-in is listed there reset the Toolbar; all Toolbar
customizations will be lost and the Toolbar will reset to what it looked
like when you first installed Outlook. To do this locate outcmd.dat and
rename it to outcmd.old when Outlook is closed. By default you can find it
here;
C:\Documents and Settings\%username%\Application Data\Microsoft\Outlook

ACT! continues to appear on the menu bar, not the tool bar, even after
renaming outcmd.dat. However, it is only on the menu bar of New Messages,
not the menu bar of Outlook Inbox. It is also on the menu bar of Word.

What to do??
