Help! How do i create an online form for my customers using a DAP?

I am starting up my own online business. But to help save time and money i
will be using an online form to colect requests from customers. These will
then be printed out in a report and followed up. I have created what i think
is a working DAP but i am unsure as to how the customers will be able to
complete the form and ensure that the data will be sent to the database, will
i have to be online 24/7?. I may have a B in ICT AVCE but this wasn't part of
the course. How do i create a link from my web design software so that the
DAP opens correctly? Another problem i have is that when the user clicks the
'Submit' button another blank form appears below the original. Why is this? I
could create a form in the web design software and make an automated e-mail
response but i feel it would be more convenient for me and the customer to
create a database.

The is a very simple method to do this which I don't think you have
mentioned. MS Frontpage. Create a form for your users (use the wizard if
you want) then set the email address to send to (again use the wizard). Save
the results as a comma deliminated file (it will go to your "private" folder
in your Fronpage web) then in your Access D Base create a macro to import the
files.

The blank page after the submit button is the response page - again create a
Frontpage page (not a form) and have a few linking fields (use the insert
button at the top of the Fronpage - design view). When your user clicks the
submit button this will hyperlink to the new page - which is merged with the
inform on the form page.

this forum mainly deals with MS Access - this would be used for anything
"after" the user clicks the submit button
