Help! Confirmation Form Error: User See Webmaster

I have a form that returns results to an email address succussfully but takes
the user to a page that states:

User: please report details to this site's webmaster.

Webmaster: please see the server's system log for more
details.

Any idea as to how to resolve? THANKS

Is the form published somewhere??
If so, is it published to a server with FrontPage extensions??
If so, did you publish using http://websitename.com??

If so, can you post a link??

The URL is www.drapecaddy.com. The form is at the Contact Us link. my web
host supports FP extensions and I confirmed its installed... even reinstalled
it.

i found out what was wrong... a form within a form. After deleting the form
TAG it worked great.
